Case Management services are available to children and adults. Case management provides the essential link between application for services and the individual actually receiving the services, and attainment of the individuals preferred lifestyle.
Case management assists with the assessment for needs and services, coordinating and monitoring the services for each individual. Case managers are also the liaison between parent or guardian and MCDS and any agency which assists the individual.
Case management teaches and monitors the individual’s rights and provides an advocacy role.
