Parents or guardians of children who have been determined eligible to receive services due to developmental disabilities may select from a list of services to meet their person’s specific needs. 

A Case Manager is generally the individual and will assist in accessing various services.  Supportive Home Care provides direct assistance and training in daily living, personal adjustment and attendant care services to enable the person with a disability to remain in a family setting.
